Eyeball #1: Found in the white vending machine.
Eyeball #2: Found on the map of the park.
Eyeball #3: After breaking the 2nd Mental Lock, float all the way to the top and find the eye against the edge.
Eyeball #4: ONLY AFTER CHOOSING TO OPEN THE GOLD SAFE! The eyeball can be found on the wheelchair after moving it.
Eyeball #5: ONLY AFTER CHOOSING TO OPEN THE SILVER SAFE❗ Once you open the safe inside while wearing Gen's mask a door will appear. DO NOT GET TO CLOSE TO LIEN❗(this will activate a cutscene and you will not be able to return). The eye can be found behind the door.
