To get the "No Time To Lose" 🏆 trophy / achievement you will have to finish the CCTV section in the "Prison Break" mission in under 4 minutes.
This section starts close to the start after getting in to the prison walls. You don't really have to do anything special to do this in under 4 minutes. Just don't hang around.
The only part I would focus on is the very end. As soon as you get in to the courtyard with the final vehicle do not wait a single second. Command Ghost to go around the back of the car and plant the bomb straight away. If you wait before doing this you are going to have to waste a lot of time while the guards are talking.
