You can find the secret Co-Op chest in Fontaine by going to the area north of the Opera. Here, look for the 2 plants in a heart shape.
Then, just stand close to the plants with a co-op partner to get the secret exquisite chest to spawn (remember that only the host of the world will be able to loot it).
