Eyeball #1: In the TV under the stairs.
Eyeball #2: Go up the left stairs to find the eyeball on a shelf.
Eyeball #3: In the corner of the red room after you press enter on the PC.
Eyeball #4: After pressing enter on the second PC. The eyeball is on the other side of a fake wall to the right of the maze image on the wall.
Eyeball #5: Found in on of the corners of the room you get teleported to after pressing enter on the 3rd PC.
